Treatments for depression

Introduction
A large number of different treatments are available for depression. New treatments (particularly medications) appear regularly. Continuing research means that the evidence for how well a treatment works is always changing too. This Fact Sheet provides only a brief summary of treatments for depression.

At the Institute we believe there are different types of depression, and that treatments should be selected according to the particular type of depression a person has.

Those types of depression that are more biological in their origins (melancholic depression and psychotic melancholia) are more likely to need physical treatments and less likely to be resolved with psychological treatments alone. Non‐melancholic depression appears similarly responsive to physical treatments (antidepressants) and psychological treatments.

Physical treatments
The main physical treatments for depression comprise drug treatments and Electroconvulsive therapy (ECT). A third physical treatment with, as yet, narrow application is Transcranial Magnetic Stimulation (TMS).

1.
Drug Treatments
There are three groups of drugs most likely to be used for depression:


Antidepressants

Tranquillisers

Anti‐manic drugs or mood stabilisers.

Antidepressants
There are a large number of antidepressants − they have a role in many types of depression and vary in their effectiveness across the more biological depressive conditions.

Selective Serotonin Reuptake Inhibitors (SSRIs), Tricyclics (TCAs) and Irreversible Monoamine Oxidase Inhibitors (MAOIs) are three common classes of antidepressants,

with the latter two having ‘broader’ actions. They each work in different ways and have different applications.

At the Institute we believe that they are not, however, equally effective− and that it is necessary to find the right antidepressant for each person.

If the first antidepressant does not work, it is sensible to move to a different kind of antidepressant. For the biological depressive disorders, more broad action antidepressants are usually more effective.

A well‐informed health provider should be able to use their assessment of the type of depression, its likely causes and their understanding of the person to identify the medication most likely to benefit their condition.

Finally, being able to decide not to use medication is important, too.

Tranquillisers
These medications are usually called 'minor' or 'major' tranquillisers.

Minor tranquillisers (typically benzodiazepines) are not helpful in depression; they are addictive and can make the depression worse. Major tranquillisers are very useful in people with a psychotic or melancholic depression where the person is not being helpd by other medications.

Mood stabilisers
These drugs are of great importance in bipolar disorder. Their use in treating mania makes them 'anti‐manic', while their ability to reduce the severity and frequency of mood swings makes them ‘mood stabilisers’. Lithium, valproate and carbamazepine are the most common.

It is important to remember that the anti−depressants and mood stabilisers are often necessary both to treat the depression that is occurring now, and to make a relapse in the future less likely. So, people sometimes need to continue taking medication for some time after they are better.


2.
Electroconvulsive Therapy (ECT)

Because of its controversial past many people feel the need to think carefully before having ECT or allowing it to be given to elatives.

Clinicians at the Institute firmly believe that ECT has a small but important role in treatment, particularly in cases of:


Psychotic depression

Severe melancholic depression where there is a high risk of suicide or the patient is too ill to eat, drink or take medications

Life‐threatening mania


Severe post‐natal depression.

While there are some short‐term side‐effects, ECT is a relatively safe and, because an anaesthetic is used, not too unpleasant.

3.
Transcranial Magnetic Stimulation (TMS)

A possible alternative to ECT is transcranial magnetic stimulation (TMS). TMS is a procedure used by neurologists, both as a treatment and as diagnostic strategy. A coil is held next to the patient's head and a magnetic field created to stimulate relevant parts of the brain. Unlike ECT, there is no need for a general anaesthetic nor is a convulsion induced.

In our view, the evidence in favour of this treatment is not yet in, but it is a major area of research at the Institute and elsewhere. If TMS is shown to be as effective as ECT this would be a distinct advance in the treatment of many mood disorders. No clear evidence about its utility is expected for a number of years.

Psychological treatments
There are a wide range of psychological treatments for depression. Some of the main ones are:

CBT, IPT, psychotherapy and counselling all provide either an alternative to medication or work alongside medication. As always, a thorough assessment of the person is needed in order to decide on the best set of approaches.

Cognitive Behaviour Therapy (CBT)

People suffering from depression ‐ particularly 'non‐melancholic depression' ‐ will often have an ongoing negative view about themselves and the world around them. This negative way of thinking is often not confined to depression, but is an ongoing part of how the person thinks about life. Many or all of their experiences are distorted through a negative filter and their thinking patterns become so entrenched tha they don't even notice the errors of judgement caused by thinking irrationally.

CBT aims to show people how their thinking affects their mood and to teach them to think in a less negative way about life and hemselves. It is based on the understanding that thinking negatively is a habit, and, like any other bad habit, it can be broken.
CBT is conducted by trained therapists either in one‐on‐one therapy sessions or in small groups. People are trained to look logically at the evidence for their negative thoughts, and to adjust the way they view the world around them. The therapist will provide 'homework' for between sessions. Between 6−10 sessions can be required but the number will vary from person to person.

CBT can be very beneficial for some individuals who have depression but there will be others for whom it is irrelevant.

Interpersonal Therapy (IPT)

The causes of depression, or our vulnerabilities to developing depression, can often be traced to aspects of social functioning (work, relationships, social roles) and personality.

Therefore, the underlying assumption with interpersonal therapy is that depression and interpersonal problems are interrelated.

The goal of interpersonal therapy is to help the person understand how these factors are operating in the person's current lifesituation to lead them to become depressed and put them at risk to future depression.

The therapy occurs in three main phases:


an evaluation of the patient's history

an exploration of the patient's interpersonal problem area and a contract for treatment

recognition and consolidation by the patient of what has been learnt and developing ways of identifying and countering depressie symptoms in the future.

Usually 12−16 sessions of IPT will be required.

Psychotherapies
Psychotherapy is an extended treatment (months to years) in which a relationship is built up between the therapist and the patient. The relationship is then used to explore aspects of the person's past in great depth and to show how these have led to the curent depression. Understanding this link between past and present − insight − is thought to resolve the depression and make the person less vulnerable to becoming depressed again.

Counselling
Counselling encompasses a broad set of approaches and goals that are essentially aimed at helping an individual with problem soving −solving long‐standing problems in the family or at work; or solving sudden major problems (crisis counselling).

Narrative Therapy
Narrative Therapy is a form of counselling based on understanding the 'stories' that people use to describe their lives. The therapist listens to how people describe their problems as stories and helps the person to consider how the stories may retrict them from overcoming their present difficulties. It sees problems as being separate from people and assists the individual to recognise the range of skills, beliefs and abilities that they already have (but may not recognise) and that they can apply to the problems in their lives.

Narrative Therapy differs from many therapies in that it puts a major emphasis on identifying people's strengths, particularly as they have mastered situations in the past, and therefore seeks to build on their resilience rather than focus on their negatives.

Self‐help & alternative therapies
There are a wide range of self‐help measures and alternate therapies which can be useful for some types of depression, either alone or in conjunction with physical treatments (such as antidepressants) or psychological treatments.

However, the more biological types of depression (melancholic and psychotic depression) are very unlikely to respond to self‐help and alternative therapies alone although these can be valuable adjuncts to physical treatments.

Why it doesn't work: We're hardwired to eat more when faced with a number of different choices. This mechanism gives us a slimming advantage when a spread consists of low-cal, filling fare such as veggies and lean protein. Why it doesn't work: If we're held to a single serving, we want to make it count. It's human nature—get your fill while you can. The problem is, if you load up your plate with a heaping pile of food, chances are you'll eat it all, says Madelyn Fernstrom, Ph.D., director of the Weight Management Center at the University of Pittsburgh Medical Center. If you're training for a sporting event, you may need to eat more to fuel your workouts—otherwise, consider regular exercise a keep-it-in-check tool, not a free pass to bulldoze your way through the mall food court.
What does work: Exercise—but not for the reasons you'd imagine. While cardio won't completely burn away a weekend binge, it does benefit health by increasing energy and lowering risk for heart disease. Another perk of busting a move? "Good behavior often begets other good behaviors," McDermott explains. Working out may make you feel good, so you eat better.

"Plan short-term goals, like 'I'm going to the gym three times this week, or I'm cooking a healthy dinner tomorrow,'" she suggests.

Soda Tax Weighed to Pay for Health Care

Taxes to pay only a fraction of the cost to expand health insurance coverage to all Americans and will face strong opposition from the beverage industry. It also could spark a backlash from consumers who must pay several cents more for soft drinks. The leaders of the Federal Senate is considering new taxes on soft drinks and other sugary drinks to help pay the costs of the reform of the nation's health care system.


On Tuesday, the Senate Finance Committee is set to hear proposals from about a dozen experts about how to pay for the comprehensive health-care overhaul that President Barack Obama wants to enact this year. Early estimates put the cost of the plan at around $1.2 trillion. The administration has so far only earmarked funds for about half of that amount.

The Center for Science in the Public Interest, a Washington-based watchdog group that pressures food companies to make healthier products, plans to propose a federal excise tax on soda, certain fruit drinks, energy drinks, sports drinks and ready-to-drink teas. It would not include most diet beverages. Excise taxes are levied on goods and manufacturers typically pass them on to consumers.

Senior staff members for some Democratic senators at the center of the effort to craft health-care legislation are weighing the idea behind closed doors, Senate aides said.

The Congressional Budget Office, which is providing lawmakers with cost estimates for each potential change in the health overhaul, included the option in a broad report on health-system financing in December. The office estimated that adding a tax of three cents per 12-ounce serving to these types of sweetened drinks would generate $24 billion over the next four years. So far, lawmakers have not indicated how big a tax they are considering.

Proponents of the tax cite research showing that consuming sugar-sweetened drinks can lead to obesity, diabetes and other ailments. They say the tax would lower consumption, reduce health problems and save medical costs. At least a dozen states already have some type of taxes on sugary beverages, said Michael Jacobson, executive director of the Center for Science in the Public Interest.

"Soda is clearly one of the most harmful products in the food supply, and it's something government should discourage the consumption of," Mr. Jacobson said.

The main beverage lobby that represents Coca-Cola Co., PepsiCo Inc., Kraft Foods Inc. and other companies said such a tax would unfairly hit lower-income Americans and wouldn't deter consumption.

"Taxes are not going to teach our children how to have a healthy lifestyle," said Susan Neely, president of the American Beverage Association. Instead, the association says it's backing programs that limit sugary beverage consumption in schools.

Some recent state proposals along the same lines have met stiff opposition. New York Gov. David Paterson recently agreed to drop a proposal for an 18% tax on sugary drinks after facing an outcry from the beverage industry and New Yorkers.

The beverage-tax proposal would apply to drinks that many Americans don't consider unhealthy -- such as PepsiCo's Gatorade and Kraft's Capri Sun -- based on their calorie content.

Health advocates are floating other so-called sin tax proposals and food regulations as part of the government's health-care overhaul. Mr. Jacobson also plans to propose Tuesday that the government sharply raise taxes on alcohol, move to largely eliminate artificial trans fat from food and move to reduce the sodium content in packaged and restaurant food.

The beverage tax is just one of hundreds of ideas that lawmakers are weighing to finance the health-care plans. They're expected to narrow the list in coming weeks.

The White House, meanwhile, is pulling together private health groups to identify cost savings that will help fund the health overhaul. Mr. Obama on Monday held a White House meeting with groups that represent doctors, hospitals, insurers, pharmaceutical companies and medical-device makers. They pledged to help restrain cost increases in the health-care system in an effort to save $2 trillion over the next decade.

"When it comes to health-care spending, we are on an unsustainable course that threatens the financial stability of families, businesses and government itself," Mr. Obama told reporters.

After working with thousands of emotional eaters, I've been able to decode the secret of overeating and break it down to reveal some basic truths.

You think the main thing you’re struggling with is feeling powerless over your uncontrollable urge to eat.

However, years of experience, have proved to me that that sense of powerlessness over food, although deeply agonizing, is really a cover-up, and the consequence, of a deeper experience of powerlessness.

1. You feel powerless about how to deal with your self-doubts.

2. You feel powerless about how to get real satisfaction in life.

3. You feel powerless to insure your own safety.

4. You feel powerless to appropriately assert your independence.

5. You feel powerless to fill yourself up when you feel empty inside.


You actually eat when you feel powerless in one or more of these five ways, because the experience of powerlessness is almost instantaneously transformed into the uncontrollable urge for food.

Let’s look a little deeper in to these powerlessness conclusions that lead people to overeat.

Conclusion #1 – The Self-Doubt Layer

Someone asks you to do something at work that you don’t know how to do. You come to the powerless conclusion that you’re stupid. To feel this way is so devastating but you don’t have to go to the vending machine and eat to avoid feeling stupid. You’ll need to learn how to talk back to your inner critic and erase the idea that the real you could be stupid.

Conclusion #2 - The Reward/Frustration Layer

You go on your 18th date from Match.com. No one feels right despite all the hope you have going into each date. You come to the conclusion that you’re defeated and there's nothing you can do about it. The search for a good mate can be disappointing but you don’t have to deal with it by stopping at a fast food restaurant on your way home. You’ll need to learn how to work on your relationships and how to get real satisfaction in life.

Conclusion #3 – The Safety Layer
You were molested or abused as a child, lived with an alcoholic or had a traumatic relationship as an adult. You come to the conclusion that you’re unsafe and can’t protect yourself. The trauma and pain you’re feeling are real but extra layers of fat can’t change what happened to you and won’t protect you from anything. You’ll need to learn how to create real safety by dealing with real issues.

Conclusion #4 – The Rebellion Layer
You’re angry with your kids for never listening. You come to the conclusion that eating is better than expressing how you really feel. You’re afraid that if you don’t stuff your feelings with food you’ll scream uncontrollably or maybe even hit them. Or your parents were controlling when you were a child and didn’t let you eat certain foods. As an adult, you eat to prove that no one can tell you what to do. You’ll need to learn the difference between childish defiance and mature assertion. You’ll also need to learn how to handle anger responsibly.

Conclusion #5 – The Emptiness Layer
You almost never have plans at night. When you’re alone you feel empty inside and can't experience fulfillment. Or you’ve recently experienced a death that you can’t get over. Or you’ve been laid off from your job. Or recently ended a relationship. You come to the conclusion that food is the only thing that can fill you up. You’ll need to learn how to feel full without food.

You can learn to overcome these five experiences of powerlessness by focusing on the fact that you are not really powerless.

You are needlessly giving away the power you do have.

Once you realize that, your cravings will be controllable. You won’t need to overeat. And what’s more, you’ll reclaim your power, not just over your relationship to food, but in all the areas of your life

What Is Weight Watchers?

Weight Watchers was founded by Jean Nidetch, a Brooklyn homemaker, in 1963. Today Weight Watchers is an international company which operates in over 30 different countries; most of them under the local language equivalent phrase to weight watchers. The Weight Watchers program includes regular meetings, learning sessions, group support, and a very effective points system.

Jean Nidetch was born in Brooklyn, New York, in 1923. She says she was an overweight homemaker - her term was ´housewife´ - who was very partial to cookies. Nidetch had taken part in several fad diets, one of which was under the sponsorship of the New York City Board of Health. She managed to lose 20 pounds (9 kilos), but was concerned her ´weak resolve´ would mean a return to her previous body weight. Nidetch got in touch with several friends and started a support group. This support group evolved and soon there were weekly classes - in 1963 Nidetch founded the Weight Watchers Organization.

In 1978, the food company H. J. Heinz bought Weight Watchers. Nidetch is still a consultant, and has set up numerous scholarship programs at the University of Nevada and the University of California (Los Angeles).

The thrust of the Weight Watcher´s program is on regular meetings, monitoring and encouragement, through self-help group type sessions. The dieter aims for a target weight or BMI (body mass index) of between 20 and 25. If your body mass index is below 20 you are considered as too thin, if it is over 25 your are overweight, if it is between 20 and 25 you are within the ideal weight range. People can aim for a BMI outside the 20 to 25 parameters as long as they have a doctor´s note saying so.

The importance of support networks

Weight Watchers Inc. says that establishing a support network at the start of any weight-loss attempt is crucial for both short and long term success. A dieter needs constant positive reinforcement. Attempting to lose weight can be a stressful ordeal for many dieters, and a support network can help make the process less daunting. Studies have found that dieters´ levels of cortisol, a hormone our bodies produce in times of mental stress, are higher when they do not join a support network. The studies also found that people who were not in support networks experienced lower abilities to pay attention and to perform tasks - these two areas of functioning tend to drop anyway when we diet - compared to dieters who belonged to support groups.

A Weight Watchers member will have regular meetings where he/she will learn about nutrition and exercise, as well as having his/her weight loss progress monitored.

Weight Watchers will accept any participant as long as he/she is at least 2.3kg (5lbs) over the minimum weight for his/her height.

Weight Watchers Points System

The points system is considered by many as the easiest tool for a person who aims to lose weight over the long term. Dieters learn how to self monitor on a daily basis - thus making themselves accountable for themselves each day.

A simple way to calculate points is (Calories + (Fat x 4) - (Fiber x 10)) / 50

Portions of foods are assigned points. If a food is high in fiber and/or low in fat it is worth fewer points. The higher the fiber content, or the lower the fat content, the more of that food you can eat each day.

Dieters can either join a Weight Watchers program online or in person.

Maintenance Period

When the dieter, or member, reaches his/her target weight, he/she enters the maintenance period. For six weeks members gradually increase their food intake until they are neither losing nor putting on weight. During these six weeks there are regular weigh-ins. If a member manages to stay within 0.91 kilos (2lbs) of his/her target weight during the six-week period he/she is then promoted to "Lifetime Member".

A Lifetime Member can attend any Weight Watchers meeting free of charge as long as he/she weighs in once per month, and does not veer out of his target weight by more than 0.91 kilos (2lbs). If a Lifetime Member veers out of his/her weight target range, he/she pays weekly for meetings and then becomes a Lifetime Member again by going through the process he/she had to go through before.

'Eruca Sativa' the Potential Anti-Ulcer Herb Medicine
Gastric ulcer affects a considerable number of people worldwide. Although the introduction of proton-pump inhibitors to the classic anti-ulcer therapy has revolutionized treatment of peptic ulcers and other gastrointestinal disorders, there is still no complete cure for this disease. It has been shown that long term use of these drugs leads to various adverse and side effects. Relapses of the malady, ineffectiveness of different drug regimens and even resistance to drugs are emerging. Thus, there is an urgent requirement to identify more effective and safe anti-ulcer agents. In recent years, Rocket "Eruca sativa L." (EER), a member of the Brassicacae family, has gained greater importance as a salad vegetable and spice, especially among Middle Eastern populations and Europeans. It is believed that plants belonging to the Brassicacae family possess diversified medicinal and therapeutic properties including inhibition of tumorigenesis, anti-ulcer, and hepatoprotective activities.

A research team led by Dr Syed Rafatullah from Saudi Arabia validated the gastric anti-ulcer properties of EER on experimentally-induced gastric secretion and ulceration in albino rats. Their study was published in the World Journal of Gastroenterology.

In this study, gastric acid secretion studies were undertaken using pylorus-ligated rats. Gastric lesions in the rats were induced by noxious chemicals including ethanol, strong alkalis, indomethacin and hypothermic restraint stress. The levels of gastric wall mucus (GWM), nonprotein sulfhydryls (NP-SH) and malondialdehyde (MDA) were also measured in the glandular stomach of rats following ethanol administration. The gastric tissue was also examined histologically. The extract was used in two doses (250 and 500 mg/kg body weight) in all experiments.

They found that the ethanolic extract of EER significantly and dose-dependently reduced the basal gastric acid secretion, titratable acidity and ruminal ulceration. Rocket extract significantly attenuated gastric ulceration induced by necrotizing agents (80% ethanol, 0.2 mol/L NaOH, 25% NaCl), indomethacin and hypothermic restraint stress. The anti-ulcer effect was further confirmed histologically. On the other hand, the extract significantly replenished GWM and NP-SH levels, as well as the MDA level significantly reduced by extract pretreatment.

They concluded that EER extract possesses antisecretory, cytoprotective, and anti-ulcer activities against experimentally-induced gastric lesions. The anti-ulcer effect is possibly through prostaglandinmediated activity and/or through its anti-secretory and antioxidant properties.

Antiviral drugs for seasonal influenza are available in some countries and effectively prevent and treat
the illness. There are two classes of such medicines, 1) adamantanes (amantadine and remantadine),
and 2) inhibitors of influenza neuraminidase (oseltamivir and zanamivir).
Most of the previously reported swine influenza cases recovered fully from the disease without
requiring medical attention and without antiviral medicines.
Some influenza viruses develop resistance to the antiviral medicines, limiting the effectiveness of
chemoprophylaxis and treatment. The viruses obtained from the recent human cases with swine
influenza in the United States were sensitive to oselatmivir and zanamivir but resistant to amantadine
and remantadine.
Information is insufficient to make recommendation on the use of the antivirals in prevention and
treatment of swine influenza virus infection. Clinicians have to make decisions based on the clinical
and epidemiological assessment and harms and benefit of the prophylaxis/treatment of the patient2.
For the ongoing outbreak of the swine influenza infection in the United States and Mexico, the
national and the local authorities are recommending to use oseltamivir or zanamivir for treatment and
prevention of the disease based on the virus's susceptibility profile.
